DVA provides white and gold health cards to veterans and eligible dependents. Occupational Therapists can assist eligible veterans to access therapy, home modifications and aids and equipment through the Rehab Appliance Program (RAP). Speak with your GP to obtain a D904 referral for Occupational Therapy.
The AT-HM Scheme will provide eligible Support at Home participants with upfront, separate funding for assistive technology and home modifications. Some items are more high risk and need Occupational therapy prescription with funding tiers ranging from low < $500, medium < $2,000 and high up to $15,000. Higher risk iems require Occupational therapy assessment to ensure items are appropriate, safe and effective.
