DVA white and gold card holders are eligible to access therapy, home modifications and aids and equipment through the Rehab Appliance Program (RAP). A GP referral is needed for Occupational Therapy.

As we age the choices we make impact our lives and are influenced by everyday activities, maintaining health and social connection. The Support at Home Program (replacing Home Care Packages) provides older adults with funding to remain living in their own home and accessing support.

• Occupational therapy assists maintaining or regaining function to live as independently as possible through: 
• Functional Assessments and recommendations
• Interventions such as Home Modification, Assistive Technology and therapies

•  Support at Home program will provide eligible participants funding for assistive technology and home modifications (AT-HM). 
• Occupational therapy assessments assist to determine needs, arrange trials, obtain quotes and access AT. 

 Assistive technology and home modifications includes:

• Mobility aides, bathroom aides such as commodes,  shower chairs and redesign to widen doorways for wheelchair access, grab rails, ramps and non-slip surfaces are examples of home modifications
