DVA provides white and gold health cards to veterans and eligible dependents.  We can assist eligible veterans to access therapy, home modifications and aids and equipment through the Rehab Appliance Program (RAP). Your GP can provide a referral for Occupational Therapy.

• As we grow older our choices have an impact on our lives and are influenced by everyday activities, maintaining health and staying socially connected. 
•  Changes in aged care through the  new Support at Home Program (replacing Home Care Packages) commences July 2025

• Occupational therapy can assist maintaining or regaining function to live as independently as possible through: 
• Functional Assessments and recommendations
• Home Modification/ Assistive Technology assessment 
• Occupational Therapy interventions

•  The AT-HM Scheme will provide eligible Support at Home participants upfront, separate funding for assistive technology and home modifications. 
• Some items are more high risk and need Occupational therapy assessment and prescription, with funding tiers ranging from low < $500, medium < $2,000 and high up to $15,000. 
• Higher risk iems require Occupational therapy assessment for safety and effectiveness. 

 Assistive technology and home modifications include:

• Mobility aides such walking frames, wheelchairs and motorised scooters
• Bathroom toileting and bathing aides such as commodes, shower chairs and bath benches
• Bathroom redesign and widening doorway access for wheelchair access and circulation space
• Grab rails, ramps and non slip surfaces 
• Home automation to open doors and control devices
